Activities of Daily Living (ADL) Assessment
Structured observation of personal care tasks including dressing, showering, meal preparation, and home mobility. Identifies where hands-on support or equipment is needed.
Navigating aged care can feel overwhelming. Knowing where to start and how to access funding is a challenge many older Australians and their families face.
Astrad Allied Health provides home-based aged care assessments across QLD, NSW, VIC, and TAS. Our registered occupational therapists come to you, assess how you function at home, and give you an accurate, evidence-based picture of your needs and goals.

Types of Aged Care Assessment
Our occupational therapists conduct a range of aged care assessments tailored to the needs of older Australians, their families, and their aged care providers. Each assessment is conducted in your home environment to ensure findings reflect your real-world function and safety.

We are committed to helping older Australians age in place safely, with dignity, and with the right supports in place. Who Benefits From Aged Care Assessment
An aged care assessment with Astrad Allied Health can support a wide range of older Australians and their families. Whether someone is planning ahead or responding to a recent change in health or mobility, a home-based OT assessment provides clarity and direction.
We identify the modifications, equipment, and in-home supports that make ageing in place safe and sustainable - from grab rails and ramps to domestic assistance and home care services.
Aged care assessment funding is available through CHSP, Home Care Packages (Levels 1-4), and DVA. Private assessments are also available. Contact My Aged Care on 1800 200 422 for guidance.
When a health change, fall, or hospital admission raises the need for support, we assess current function and recommend the services and equipment that restore confidence and safety at home.
Our assessments and reports are structured to support service planning, home modification approvals, and funding applications. GPs and aged care providers rely on our clinical findings to guide next steps.
We give families and carers an objective, clinical picture of current function and needs - the information they need to plan ahead, access funding, and make informed decisions together.
Eligible veterans and DVA recipients can access aged care OT assessment through the Department of Veterans' Affairs. We deliver DVA-funded assessments and reports that meet DVA standards and requirements.
Our aged care assessment process is structured, thorough, and centred on your goals. We follow a clear pathway from referral through to written report and recommendations.

We accept referrals from GPs, My Aged Care, aged care providers, hospitals, and families. We discuss your current challenges, daily routines, and goals to prepare for the assessment.
Our occupational therapist visits you at home to observe how you manage daily tasks, identify hazards, and understand what supports would make the greatest difference to your independence.
You receive a detailed written report with clinical findings, functional observations, and tailored recommendations. Reports support Home Care Package planning, ACAT/ACAS processes, and home modification approvals.

An aged care assessment is a professional evaluation conducted by a registered occupational therapist to understand how an older person is functioning in their daily life. It differs from an ACAT/ACAS assessment, which is the government-administered process that determines eligibility for aged care services.
An OT aged care assessment looks specifically at functional capacity – how a person manages personal care, mobility, domestic tasks, and community participation. It produces clinical findings that support service planning, home modification approvals, and funding applications. The assessment is conducted in the person’s home, where real challenges and real solutions become visible.
Both types of assessment work together. The ACAT/ACAS opens the door to aged care funding, while an OT functional assessment shapes what those services and supports look like in practice. Understanding this distinction helps families navigate the aged care system with greater confidence and clarity.
